Neighborhood Team Member Agreement

We welcome you as a community member who resides in one of our targeted communities (71106 or 71107). With your help, we can further strengthen our families and support our communities. By completing the information below, you are agreeing to partner with MCC and attend a monthly meeting (virtual or in person), as your time permits. At any time, you may withdraw your involvement with the MCC program.
